If you are selected as a Vendor to work with the 91��ɫ, you will be asked to submit your company information through 91��ɫ's Vendor and Individual Payee Registration Form.

You will also be asked to provide a Certificate of Insurance showing General Business Liability Insurance, Automobile Insurance, and Worker’s Comp Insurance on the certificate. Colorado Seminary which owns and operates the 91��ɫ should also be listed as the additional insured on the Certificate of Insurance.
